This policy covers how Smart Choice Medical Travel treats personal and financial information that Smart Choice Medical Travel collects and receives in accordance with the Data Protection Act.

1. Information we collect

The personal information that Smart Choice Medical Travel collects from you is only that required by Smart Choice Medical Travel or any partner medical service operator (clinic, doctor) to provide you the information, products or services requested.
The requested personal information includes your name, date of birth, address, phone number, email address.
For some services we might also ask for information about your previous medical treatments and past or current medical conditions as well as travel plans. It is your sole responsibility to ensure that the information provided is accurate and to immediately notify Smart Choice Medical Travel of any changes to the information provided.

Financial information

When you pay for Smart Choice Medical Travel' s services or Smart Choice Medical Travel's medical partner's services using your debit/credit card we ask you to provide information that is necessary to process the payment. This information includes your credit or debit card number, security number, name and debit/credit card billing address. Smart Choice Medical Travel will not store any of the provided information in any form.

2. How the information is used

The information provided by you will be used or disclosed only for the purpose for which is given and forwarded directly to the party it is intended for or required by law.
You are covered by the Data Protection Act which means any information you give us won't be forwarded to third parties. Smart Choice Medical Travel does not trade, rent or sell client details to third parties. Should Smart Choice Medical Travel's intentions change it will be done only with your authorisation. Smart Choice Medical Travel will always comply with any data protection legislation currently in force.

3. Customer Service Correspondence

If you send us correspondence, including emails and faxes, we retain such information in the records of your account. We will also retain customer service correspondence and other correspondence from Smart Choice Medical Travel to you. We retain these records in order to keep records of our relationship, to measure and improve our customer service. We may, over time, delete these records if permitted by law.
